How to Enroll

Enrollment at MCPA
We guide families through each step of enrollment.

Starting at MCPA is a conversation, not a form. We walk with families through each step so the transition is smooth, respectful and centered on the participant.

Step 1 — First phone call

Call our Edina office at (952) 943-2023 or email [email protected]. We’ll answer your questions about program hours, cost, locations and funding options – usually in 15–20 minutes on the phone.

Step 2 — Tour a location

We encourage every family to visit at least one of our four Minnesota locations (Edina, Eagan, New Hope, or St. Paul) before making a decision. Tours are free and typically include 30 minutes with our program director, a walk through the art and music studios, and time to meet current participants and staff.

Step 3 — Application & interview

Complete a short written application and schedule a one-hour personal interview for your loved one. The interview helps our team understand interests, abilities, medical needs and social goals.

Step 4 — Physical exam & medical review

A current physical exam with the participant’s primary care provider is required. We’ll send you the exact form and can fax it to the clinic directly.

Step 5 — Care plan & start date

Our program director writes an individualized care plan based on the interview, medical review and family input. Together we choose a start date – usually within 2–3 weeks of the completed application.

Funding support. We accept CADI, CAC, DD, MR/RC, TBI, EW and VW waivers, Veterans Administration benefits, long-term care insurance, HMOs, and private pay.
